Personal Care Assistance is a health care program that provides services to individuals with differing abilities and medical needs. The main focus of the program is to help promote the recipient's independence so they can remain in their homes and community.
We provide a variety of services such as Elderly Waiver (EW), Alternative Care (AC), Brain Injury (BI), Developmental Disabilities (DD), 245D Services, etc.

Care plans are developed by our Qualified Professional Supervisor. Care plans will be based on the client's initial assessment for services. Our QPs also complete supervision visits every 90-120 days. Care plans can be revised as necessary according to the client's needs to ensure care is up to quality.
All information is confidential and cannot be shared without consent. Our agency has annual trainings to keep our employees informed about the importance of HIPPA laws and violations.
If services provided by the PCA are not up to quality or meeting the client's needs, the client has the option to change caregivers. We will provide another caregiver to the client as soon as possible. When a client needs to confirm a caregiver's work history, we will provide the client with that information as necessary.
In some cases (especially for new clients) , a doctor's referral is necessary. The doctor's referral should state why the individual needs PCA services.
If our office hours are closed, please use your best judgment of the emergency and dial 911 as needed. When office hours resume, all emergencies must be reported to our office staff immediately.
All caregivers must pass a background study. PCAs will need to complete the Minnesota Department of Human Services PCA Training Test. All employees are required to complete a TB screening at initial hire. Any records of Hep B, Covid, and other vaccinations are recorded. Supervision visits are completed every 90-120 days as necessary. In addition, all caregivers have required annual trainings to complete.

Yes, all care plans are created prior to the caregiver's start date. Client's with complex needs will be assigned to one of our RNs.
